A class for anyone with diabetes, prediabetes, or at risk for diabetes

Learn how YOU can make a difference in your blood glucose levels and reduce the risk of complications of diabetes. This 4-part series helps pull skills and knowledge together and put it all into action!

4 classes starting April 4th

Yes, you can make a difference in your blood glucose (sugar) levels. You can reduce the risk of complications of diabetes with exercise you enjoy, healthy eating, taking medication and mindful coping.

The 4 part Take Heart! education series helps pull skills and knowledge together to put into action with goal setting, problem solving and monitoring. The class, led by a Certified Diabetes Care and Education Specialist (CDCES), is interactive with the use of Conversation Maps and lasts approximately 2 hours. If you have diabetes or pre-diabetes, or are at risk to develop diabetes, this class is a MUST!

Class 1: On the Road to Better Managing Your Diabetes

  • What is Diabetes?
  • Common Myths about Diabetes
  • Emotions and Diabetes
  • Dealing with high and low blood glucose levels
  • Managing diabetes by eating well, keeping active, and medication
  • Importance of having a plan and engaging a support network and healthcare team.

Class 2: Healthy Eating

  • The relationship between blood glucose and food.
  • Different food groups and nutrients in food
  • Focusing on what, how much, and when you eat, as well as challenges that may arise.
  • Meal planning and other strategies for healthy eating.

Class 3: Monitoring

  • What blood glucose and insulin are
  • Blood glucose targets
  • Monitoring and knowing your A1c
  • What can make blood glucose go up and down and prevent highs and lows.
  • Using the results to manage your diabetes

Class 4: Continuing the Journey

  • The natural course of diabetes
  • Recognizing the fact that it may become more difficult to keep your blood glucose within your target range
  • Potential long-term complication
  • How to prevent, delay or reduce complication by keeping your blood glucose on target
  • The importance of the ABCs

It has been 20+ years since I was diagnosed with Type II Diabetes. This four week program, I must say, expanded my knowledge of diabetes by at least 200%. The format and class organization was simply excellent. Additionally, the “encouraged” participation by my fellow classmates not only contributed to the program content, but significantly drove me to take more control of my diabetes management.
